Rob Page’s Wales got off to a slow start, but Neco Williams of Nottingham Forest saw a shot go over the crossbar a few minutes later before Iran’s Ali Gholizadeh put the ball in the back of the net.
The first goal was disallowed due to offside, but Iran kept up the pressure in the second half.
One-on-one with Wayne Hennessey, Sardar Azmoun shattered the post before his teammate did the same.
With five minutes left, the goalkeeper was subsequently sent off for running out and high-kneeing Mehdi Taremi.
Iran took the lead in stoppage time thanks to a long-range shot from Roozbeh Cheshmi, and Danny Ward was beaten by Ramin Rezaeian to secure the three points.
